Prepare for Performance Review
A supervisor can use a standard performance evaluation form, such as the one below, to help him or her to prepare for the actual evaluation. The form can be used as a guide when collecting pertinent information and preparing notes. The form used should, of course, be the same one that will be used during the actual evaluation session.
